A request may include a name, phone number, optional email, service address, town, ZIP code, property and damage type, whether the damage is still active, a description, optional notes and consent. Technical records may include request time, browser information, the referring page, limited campaign-source fields and a one-way network identifier used to prevent abuse. Do not submit policy numbers, payment cards, medical information or Social Security numbers through the initial form.

How information is used

Information is used to respond to the request, communicate about the requested service, share relevant details with an independent restoration company selected to respond, prevent fraud, maintain records and improve the service.

Service providers and restoration companies

Relevant details may be shared with an independent restoration company and with vendors that support hosting, forms, communications, analytics, security and customer management. Information should be limited to the work each recipient performs.

Security

Administrative access is restricted, request delivery is authenticated when a receiving system supports it, and automated abuse controls are applied to the form. No internet system can guarantee absolute security. Please do not include sensitive information that is unnecessary for restoration service.

Retention

Service-request records are generally retained for up to 24 months so the request, delivery and follow-up can be documented. Abuse-prevention records are generally retained for up to 90 days. A record may be deleted sooner when it is no longer needed or kept longer when required for security, a dispute, legal compliance or an active business record.

Access, correction and deletion

You may request access to, correction of or deletion of your request information using the contact method published on the Contact page. A request may require identity verification, and some records may be retained when legally permitted or required.
